Daily Check-in No Longer Required
As of May 24, 2021, Â鶹ӰÊÓ no longer requires you complete the daily check-in form when visiting campus locations. In lieu of completing this form, please take a moment to assess your health.
Please be sure that you and anyone whom you have been in close contact with (closer than 6 feet for 15 minutes or longer) have not tested positive for COVID-19 and are free from any of the following COVID-19 symptoms not explained by other reasons (e.g. allergies) or common to what you usually experience:
- A 100 Degree (F) or Higher Fever
- A Cough
- Shortness of Breath or Difficulty Breathing
- Chills
- Repeated Shaking with Chills
- Headaches and/or Body Aches
- Muscle Pain
- Recent/Unexplained Loss of Taste and/or Smell
- Sore Throat
- Nausea, Vomiting, and/or Diarrhea
